Near-Miss Reporting Form
Report Near-Miss Events using the following form:
Researchers will not be penalized for Near-Miss Events. The reports are visible to the members of the safety committee and will be discussed in anonymized form at the faculty meeting. They are used to improve safety procedures. The idea here is that if we address all small incidents, the likelihood of occurrence of severe incidents strongly decreases.
